Hello,
I am filling out my medical pre-screening paperwork for MEPS. When I was 10 my parents split up and I havent spoken to or seen my biological father since. Upon my mother's remarrying to my stepfather, I was adopted by him, took his last name and all of my paperwork (Birth Certificate, SSN, etc.) has my current last name on them. My question is, whom should I list as my father? Should I put him as my stepfather or father considering I was adopted by him and all my paperwork has his name on it? Should I list my Biological Father?
Thank you.

You should have a new birth certificate with your adopted fathers name on it. He is your father...put down his name.If it asks for alias give the last name you had before you were adopted.

Ok, and one of my good friends is going in on the buddy program with me and he is in basically the same situation, but he already submitted his paperwork and our recruiter told us it doesn't matter. We are supposed to go to MEPS tomorrow. How can HE CHANGE IT?

Well, now I am quite confused by both of your conflicting replies...I want to ask my recruiter but since he told me to omit it, I am kind of skeptical...

xxjupiterxx1,
As cantonmark stated you may have to list you biological father somewhere on the form or paperwork. But the day you were adopted your name was changed and your adoptive father is now for all intents and purposes your father. If you were not adopted and he and your biological mother were married he would be your step father and you then would have to list your biological information on the form.
Look at your new birth certificate as the one when you were born was null and void and see whom it lists as your father. That is who you put down.
Again for nicknames, alias ect. You list the last name you had before adoption...this is for background and FBI checks. You are no longer Chris ? Your name is I guess by birth certificate Chris James so Mr. James is listed as your father.
Tell your friend to tell the recruiter he needs to make a correction before the paperwork is submitted.

Yes, my birth certificate states "Christopher James" and lists my "step-father" as my father...Thank you very much for clearing that up for me. Also, along the lines of the FBI investigation purposes, my recruiter said I did not have to list any alias because my name was change so long ago and before I was 18...is this also true?

Can't say what the regulation is now. I have a nephew whom was adopted before he was 18....around 10 or so and he had to list his last name before adoption on the form so they could do a background check.
This was about 7 years ago.
I don't know if you have to or not but I would say so...because that was a name you used to go by. I am not certain how it works or if this only pertains to security clearance or not.
