Is it difficult to request and receive this surgery (PRK) after one has finished boot and SOI?

It does depend on your command. When I was about to get out they offered this free surgery All that was required was you had to have 2 years left on contract

There's an actual form you need to fill out. Asks for your name, social, command, ECC and EOS. Then you ahve to run that up your chain of command and your OIC has to rate you from a scale from 1-5 (i think) on your priority level (1 being the highest priority). Your OIC then signs it, then you take that form and you need to take it to a Naval Optometrist (make an appointment first). Then she fills out the last portion of that form to see if you are eligible. Finally you have to fax or scan it to a Naval hospital that performs the PRK procedure. I'm in Norfolk, VA so I sent mine to Naval Medical Center Portsmouth, VA.
Hopefully everything works out. The wait was about a year long and I couldnt wait so I opted to pay out of pocket and go with LASIK.
