Does anyone know where to get the Commemorative US Flag Raising Request Form for requesting a flag that has been flown over the Marine Corps War Memorial?

Evedently the form I am looking for is an enclosure to the below policy memorandum. I guess I will call them on Monday.

Subj: MARINE CORPS WAR MEMORIAL COMMEMORATIVE FLAG RAISING
PROGRAM

1. Purpose. To publish guidance, information, and areas of responsibility for the subject program.

2. Information

a. The Marine Corps War Memorial on Arlington Ridge in Arlington, Virginia, is owned and maintained by the United States National Park Service. The National Park Service ensures that the monument and surrounding park are well maintained, clean, and free of debris. Additionally, the National Park Service ensures that the flag of the United States of America that is flying over the memorial is serviceable.

b. The Henderson Hall commemorative flag raising program was instituted to provide a means for individuals to have the U.S. Flag flown over the Marine Corps War Memorial in order to honor or recognize special events such as retirement from the Armed Forces or the commissioning or christening of a ship.

c. Special consideration should be taken in requesting a commemorative flag raising that the occasion for the request is commensurate with upholding the sacredness of the Marine Corps War Memorial. Unmonitored or unrestricted use can devalue the entire program or run the risk that it will be used for an occasion that is not worthy of Marine Corps recognition. Some occasions for proper consideration are:

(1) A retirement or release from active duty in the armed forces after honorable service.

(2) For a veteran who has served honorably.

(3) Through a special request from a public official.

PROGRAM

(4) For a special event.

d. Approval authority for acceptance of a flag for this program lies with the HQBN Adjutant, S-1 Officer or Executive Officer. Ultimately, the Commanding Officer has the final decision in any contested acceptance consideration. The criteria for acceptance will be based on those instances outlined in subparagraph 2.c above. All other requests outside the above guidelines will be reviewed on a case by case basis.

3. Action

a. Adjutant

(1) Accept and process request forms for flag raisings. See enclosure (1). Ensure request forms are filled out legibly and in full to ensure timely flying of the flag and proper certification of the event.

(2) Ensure flags to be raised are turned in to PMO along with all pertinent information necessary for the event.

(3) Produce Flag Certificates commemorating the occasion for which the flag was flown. Occasions for the flying of the flag fall under two general categories:

(a) "In honor of" a person, group, or special event.

(b) "In recognition of" an achievement.

(4) Prepare the flag and certificate and return to the submitter.

(5) Certify the Flag Certificate by witnessing the Executive Officer's signature and inscribing the certificate with the Marine Corps Seal.

b. Provost Marshal

(1) Transport the OOD and the flags to the Marine Corps War Memorial.

(2) Fly the flags. Flags are not flown during inclement weather (rain, snow, or high winds) due to the danger involved in ascending the War Memorial to fly the flags.

c. Officer of the Day

(1) Oversee and witness the flag raisings, and note in the OOD's logbook how many flags were raised on a particular date.

(2) Report on the condition of the grounds surrounding the Marine Corps War Memorial and note in the OOD's logbook these conditions.
