VA form 21-4192

I am working on my PTSD claim to the VA along with my IU claim. I have a question about VA form 21-4192 (the form your employer fills out).
My question is who sends this form to the employer? Do I fill send it to my former employer and have him send it directly to the VA or will the VA generate this form based off of my PTSD claim and send it to him automatically?
Anyone who has accomplished this task recently would be of great help.

You fill out section 1. and send it to the VA so they know who to send it to.

The VA sends it to your employer to fill out section 2 and 3 and your employer sends it back to the VA.

I have the form right in my hands. Underneath section 1. it tells you and your employer complete instructions on how to fill it out and if they have questions there is a number listed.
